Dr. Loan Lam is a current first year pharmacy resident at Providence Visiting Nurses Association/Washington State University CPPS training with an emphasis in geriatrics under the guidance of Dr. Brian Gates and Dr. Jeffrey Clark. In her unique practice in home healthcare, she plays a pivotal role on the pharmacy team commanding pharmacy services to coordinate care by responding to complex medication-related concerns and managing high-risk chronic disease states within a multidisciplinary team (skilled nursing, physical therapists, occupational therapists, registered dietitians, masters in social work). Secondarily, she spends much of her time in her residency to foster and empower APPE student pharmacists through academia and mentorship.

CE INFORMATION

1 hour of ACPE accredited CE

The Washington State Pharmacy Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.

To Earn CE Credit
You will be required to log into the gotowebinar, attend the full presentation, and complete a post-activity evaluation form to receive credit. The evaluation form will be included as an attachment in the webinar. After the evaluation form is completed, you must return it to Maria Lieggi (maria@wsparx.org) at the Washington State Pharmacy Association (WSPA) to have your CE uploaded.

WSPA will upload the CE credit to the CPE Monitor. WSPA only has 60 days to upload the CE from the initial date of the training, so it is recommended that attendees check their CPE Monitor 1-2 weeks prior to the 60 day deadline in order to verify the status of their credit for presentations attended. After the 60 day deadline, the CE cannot be uploaded to the CPE Monitor.
